  Fall Confirmation Date for RE Conf. 2/8th grade school students : TBD - The date will be posted as soon as it is assigned by the Diocese of Joliet.
  All forms are due no later than noon on May 1, 2026.
  Completed forms can be e-mailed to [email protected], dropped off in the RE dropbox (located to the right in the Ministry Center vestibule below the monitor) or mailed to:

- Confirmation Record Form Please fill out the form in its entirety so the sacrament can be properly recorded in church ledgers.
- Info About Your Confirmation Sponsor Requirements for being a sponsor
- Sponsor Eligibility Form The sponsor should fill out this form and have it signed by his/her home parish.
- Sponsor Interview Form To be completed by the candidate and sponsor together
- Letter to the Pastor To be completed by the candidate
- Confirmation Saint Name Report To be completed by the candidate once they have selected their Confirmation name

- Food Drive Collection Confirmation Project Required service project for all students preparing for Confirmation
